I am no real comic expert, but I read both Batwoman comic runs and some other Batwoman comics.
In the comics, there are quite a lot of fantasy/horror elements like monsters, ghosts and stuff like that.
Do you think they will have such fantasy elements in the show?
From the little we saw so far, I would say no, but I haven't heard anything about it from official sources.
I would like the show to as "realistic" as possible.
No magic, no monsters.
I wasn't very happy about these elements in Arrow. (But I still really like the show nonetheless)
What do you think they will do?

From what the creators have been saying, and since Batwoman will replace Arrow, I think they are going to keep things more grounded and street level.
Flash, Legends, and Supergirl deal with the more magical stuff already. Of course, being a Bat-Family-Hero, they can't avoid some level of magic and supernatural since characters like Poison Ivy, Mr. Freeze, Killer Croc, and Clayface aren't exactly realistic (or are they considered more sci-fi?). But I think they won't take it as far as the other shows, that have heroes with actual superpowers.

Since BW comes from the Bat-verse, it will have a look and feel closer in comparison to the Dark Knight trilogy than the other shows. And, it is also likely that elements of horror and fantasy may come into the mix. It will be very interesting to see where it goes.

Yes, it should definitely be more spooky than the other shows. There are Gotham rogues who are serial-killers, mad-scientists, and evil-cultists, so the villains in general may be more sadistic and/or creepy.
Also of note, most of the Flash's villains start around the same time as him after the particle accelerator. Supergirl had one or two of Superman's villains (as far as the Arrowverse version of the story goes), but most of her rogues came with her from Fort Rozz and became active once she started her career. It also helps that she protects a different city to Superman.
Batwoman by contrast, looks like she will be inheriting a fair few of Batman's villains, who are already established, going by the names on the cells at Arkham Asylum.

Given the whole Alice story angle and the Religion of Crime, I don’t see how fantasy elements wouldn’t be involved. Considering Legends has had such a huge emphasis on magic and magical creatures, specially with incorporating Constantine, within the last couple of seasons, I wouldn’t put it past them. I could even see a few crossovers between the two just on the magical creatures aspect alone. I personally think Legends has overdone it on the magical references a bit.
I mean, the whole Time Jumping Dr Whoesque storyline made some sense in the beginning for the story they were running with Vandal Savage and Rip thing but then it just devolved into... I dunno.. weird. The show just feels campy now. So, with that, I can see where Siggi’s fears would be with the incorporation of magic stuffs. As Bratty Bat pointed out, there are some characters that have a more magical feel to them already but even then their connections are usually explained by some chemical or other scientific mishap. I guess I’m of a mind that as long as it makes sense to have the magic stuffs, then, I guess we’ll see how it goes.
